Benchmark Health Partners, LLC values your privacy. This Privacy Policy explains how we collect, use, share, and protect information when you visit benchmarkhealthpartners.com and when you use ADI services that are linked from the Site. It also explains the choices available to parents and families.

This Policy covers information collected through the Site, including forms, newsletter signups, and other pages on benchmarkhealthpartners.com. If you enroll in ADI, additional information may be collected through the ADI experience to provide monitoring and guidance. This Policy applies to that information when it is connected to your use of ADI through the Site.

  • Contact and support messages you send to us
  • Information you submit through web forms


ADI membership information

If you choose to create an ADI membership, we may collect information needed to set up and support the experience, including:


Parent or guardian account information


  • Youth athlete profile information provided by a parent or guardian, or provided by the athlete with parent or guardian approval
  • Check in responses and other inputs submitted during ADI use
  • Communication preferences


Information collected automatically

We may collect:

  • IP address, browser type, device type, and basic device identifiers
  • Pages viewed, time spent, referring pages, and interactions
  • Cookies and similar technologies, as described below


3. How We Use Information


We use information to:


  • Provide and support the Site and ADI services
  • Respond to questions and support requests
  • Improve performance, reliability, and user experience
  • Communicate updates and product information you request or opt in to receive

4. Youth Athlete Information and Parent Controls


  • ADI is designed for families with youth athletes ages 10 to 18. A parent or guardian is expected to manage the account and provide permission for information submitted about a minor.
  • We design the ADI experience to support parent control in practical ways:
  • Parents can review information submitted for their athlete
  • Parents can decide what to share and when to share it
  • Parents can request access, correction, export, or deletion of information as described in Section 9


5. Cookies and Tracking Technologies


We use cookies and similar technologies to support core site functionality and to understand how the Site is used.

  • Essential cookies that support navigation and form submissions
  • Performance and analytics cookies that help us understand site usage and improve content

For youth athlete accounts, a parent or guardian may exercise these rights on behalf of the minor. To make a request, contact us using the information in Section 12.


10. Health Related Information


Some information used in ADI may relate to athlete health and participation. Benchmark Health Partners does not provide medical treatment through this Site. If you choose to submit sensitive information through contact forms or email, we treat it carefully and use it only to support your request and deliver the services you asked for.


11. International Visitors


If you access the Site from outside the United States, your information may be processed in the United States or other locations where our service providers operate. We use safeguards designed to support lawful transfers and appropriate protection.


12. Children’s Privacy


The Site and ADI services are designed for families. ADI is intended for youth athletes ages 10 to 18 with parent or guardian involvement.


We do not knowingly collect information from a child without a parent or guardian’s permission. If you believe a child has provided personal information without appropriate permission, contact us and we will take steps to address the situation.
